By engaging the services of Ocean Code Home Care LLC. you agree to abide by these
Terms and Conditions. If you do not agree with any part of these terms, please refrain
from using our services.

2. Services
a. Service Description: Ocean Code Home Care LLC. provides non-medical home care
services, including but not limited to assistance with daily living activities,
companionship, meal preparation, and other services as specified in the care plan.
b. Customized Care: Care services are tailored to the individual needs and preferences of
the client. The care plan is developed collaboratively to ensure client satisfaction.
c. Caregiver Assignment: We match clients with qualified caregivers based on client
requirements, caregiver skills, and compatibility.

3. Client Responsibilities
a. Accurate Information: Clients must provide accurate and complete information
about their health, medical history, and care requirements.
b. Communication: Open communication with caregivers and the agency is essential for
effective care delivery.
c. Payment: Clients are responsible for making payments for services as agreed upon in
the payment schedule.
d. Care Plan Adherence: Clients must adhere to the care plan and promptly inform the
agency of any changes or concerns.
e. Feedback: We encourage clients to provide feedback about our services to help us
improve and meet their expectations.

4. Payment
a. Billing: Clients will receive invoices for services rendered, and payments must be
made in accordance with the agreed-upon payment schedule.
b. Rate Changes: Rates are subject to change with advance notice.

5. Termination of Services
a. Reasons: Services may be terminated for reasons including but not limited to the
completion of care goals, changes in client circumstances, or failure to meet
payment obligations.
b. Notice: The agency reserves the right to terminate services with adequate notice,
and clients also have the right to terminate services with the specified notice
period.

6. Privacy
a. Data Protection: We are committed to protecting the privacy of client information and
adhere to applicable data protection laws and regulations.
b. Consent: Clients consent to the agency's use of their personal and health information
for service provision and administrative purposes.

7. Dispute Resolution
a. Mediation: Disputes arising from these terms will be resolved through mediation or
arbitration in accordance with applicable laws.
b. Legal Recourse: Clients have the right to pursue legal recourse if disputes remain
unresolved.

8. Amendments
a. Modifications: We reserve the right to modify these Terms and Conditions. Updated
terms will be posted on our website with the effective date.

9. Miscellaneous
a. Governing Law: These Terms and Conditions are governed by the laws of
Massachusetts.
b. Entire Agreement: These Terms and Conditions constitute the entire agreement
between Ocean Code Home Care LLC/ and the client, supersedes all prior agreements, and
may only be modified in writing.
c. Severability: If any part of these terms is found to be invalid or unenforceable, the
remaining provisions remain in full force and effect.
